Appointments, Referrals & Payments

Hobart Diabetes Health (HDH) offers an individualised approach to diabetes care. 

There are many factors that influence what your current needs are in managing your diabetes. HDH endeavours to work with you using best practice guidelines and modern technology to support you and your goals. 

At your appointment, HDH can provide you with:

  • A better understanding of your diabetes Diet and exercise recommendations
  • New blood glucose meter (glucometer), including blood ketone meters
  • Registration or upgrading your National Diabetes Service Scheme (NDSS) status
  • Education regarding oral-hypoglycemics and injectable medications
  • Regular support and guidance throughout pregnancy and pregnancy planning

HDH can also discuss with you:

  • Insulin Pump Therapy
  • Patch Pump Therapy
  • Continuous Glucose Monitoring Therapy (CGMS) & assess NDSS subsidy eligibility
  • Flash Glucose Monitoring System
  • Insulin bolus calculators 

Referrals and Payments

Hobart Diabetes Health is a private practice and therefore charges a fee for all consultations. The fee must be paid before, or at the time of your appointment. 

Appointments can be made via website booking, email or phone. You do not need a referral from your GP to book an appointment however, you may wish to ask your GP for a ‘Chronic Disease Management Plan’ (Item 721) or ‘Team Care Arrangement’ (Item 723) that provides a Medicare rebate for a maximum of 5 Allied Health Care visits per year.

If you have private health insurance ‘extras cover’, you may be able to claim a rebate from your insurer. Please enquire with your health care fund for further information.

Payment

Hobart Diabetes Health accepts EFTPOS, cash or credit card.

Cancellations

24 hours notice is required for appointment changes. If cancellation or rescheduling is required within 24 hours of your scheduled appointment, a 20% cancellation fee will apply.
